Insurance
and Payment Information
We work closely with the listed insurance
carriers, so we can bill these carriers for you. However, you are responsible
for any co-pays or deductibles on the day of service. Also, we do not participate with other
commercial insurance companies or HMO's. If you have one of these insurance
plans, we ask that you pay for services when care is rendered. We will
provide you with an itemized receipt to submit to your insurance company
for reimbursement. If you have questions about our fees, we will be
happy to discuss this with you. Credit and debit cards are also accepted.
Appointments
We understand that your schedule
may change and you may need to cancel an appointment. Please call our
office 24 hours in advance to cancel an appointment at (231)
935-0440.
Prescription Refills
Please call our office with the prescription
that you need refilled. Prescriptions will be called in within 24 hours
if possible.
Test Results
Our office staff does not provide
information to patients regarding the results of diagnostic studies
or laboratory tests. If you wish to talk to the physician about test
results, please leave a message and one of our physicians or nurses will call
you. Some test results are available the day after your study or blood
work is taken; others may require a week or more to complete and communicate. The nursing staff at Pulmonary & Critical Care is the initial phone contact for patient questions.
